Hair Transplant on scar after hairline lowering surgery - Should I go for FUT or FUE?

Hair transplants or excision of the scar with proper closure with the Irregular Trichophytic closure? You don’t been grafting -only revision. This is a procedure I developed 40 years ago. This leaves an undetectable scar(see below). Transplants don’t grow well in scar tissue!

The previous scar seems to have been closed under tension as a result of which it has become like this. There are two ways to go about it: 1. Scar excision and having a thin linear scar in which transplant can be done.2. Direct transplantation into this scar.Since the requirement of grafts is less, you may go in either FUT or FUE from an experienced hair transplant surgeon

Hello and thank you for your question. Yes you can cover up this scarring with a transplant. In my opinion a FUE would be your best choice. I do advise you to get a very experienced doctor to do the procedure, you don't want/need further complications by someone not doing it correctly. Best wishes.Dr. T

This is not a typical result after hairline lowering surgery, particularly how wide the scar is on the right side.  It is my recommendation that you first undergo a revision hairline advancement to achieve a much finer line scar, then as soon as three months later, if there is still scarring, at that point grafting either through FUE or FUT can be performed, that will also allow for more rounding out if desired.

I've never seen aa scar like this after the Irregular Trichophytic Hairline Incision technique that I developed and described many years ago. I suspect this is secondary to closing the wound under too much tension. I recommend an excision of the scar and a refined closure for a quicker and more natural result. It will be very difficult to achieve natural hairline density with grafts in this large scar.

This is a highly unusually bad scar. I say unusual, because incisions that are horizontal on the forehead tend to heal well. I suspect that this man had either unusual tension as the advancement was closed by the surgeon (most probable cause) or this particular patient had scarring tendencies.  This can be fixed with hair transplants, either FUE or FUT; however considering the degree of scarring, FUE might be a better choice. The surgeon who does this better have a great deal of experience when designing the hairline to repair this problem as a poorly designed hairline will just add to this man's misery.
