55, 5'7 - 140#. Currently UTM 550/500 mod teardrop implants w/ crescent lift 19 years ago. Would like to downsize significantly to align with my lifestyle. I don't know if I need a lift, am considering a low profile. I don't want projection something that feels natural to my build. I feel very top heavy and look overweight with clothes.

Thanks for your question! If you are going to replace to a lower profile, I would advise doing the lift again. The good news is that you should get a very nice, long-lasting result from a secondary procedure since your first lift held up well. Best of luck!

breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.
Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.
While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.
If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.

If you downsize substantially you'll likely need a breast lift as there will be a lot of skin.  I'd suggest a breast lift and replace your implants with something around 250cc.  This would give you a beautiful perky and smaller result.

The technique I recommend is explantation and lift using The Bellesoma Method.  The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.  Later, fat transfers can be performed if additional volume is desired.

I suggest meeting with a plastic surgeon and discussing going down in size, tightening your lateral pockets, and a revision of the lift might be needed. You might do well with volume of 350-400. I am not a fan of low profile implants because they are so flat. also, I prefer smooth round implants for best long term results. A moderate or moderate plus profile is usually very natural in appearance.
