3 years post revision rhinoplasty - Is gore tex removal medically necessary? (Photos)

Hi! I had a revision rhinoplasty almost 3 years ago and my surgeon used gore tex implants. I‘ve struggled with recurrent swelling, chronic congestion after 2 rounds of injections (at month 5 and month 11). Post injections my nose thinned out a low bridge thinned out a lot to thr point of seeing bone and has sincr widened again. I’m looking into one final revision to have the gore tex removed and replaced with rib graft recommended in numerous Consultations). A few surgeons have expressed they suspect these issues are due to foreign body reactions. So my question is, how is this deemed medically necessary (if at all). I’m hoping insurance can partially cover the procedure as I can’t afford out of pocket payment at this time but really want better quality of life. Additionally, I’ve had consults discussing auto versus allo rib grafts. Given this history, are there significant benefits versus cautions to either one? For context: gore Tex implants were placed bilaterally on the sides in the mid area. The top row of photos is after the second steroid injection about one year post op. The bottom row of photos were taken yesterday and today (a few weeks short of 3 years post op).
