I was advised to lose 20lbs before proceeding with a BBL and fat transfer due to deep visceral fat, is it possible to have a safe and successful procedure as well as decent results? I do understand because of deep fat I want have a really small waist, however is it possible to get rid of the really big flanks?

Answer: The best procedure for you The protruding appearance of your abdomen is because you have visceral fat, subcutaneous fat, saggy skin and loose muscle. So you must lose the weight to reduce visceral fat and after that automatically your abdominal subcutaneous fat will reduce, but liposuction will not improve the saggy skin and either the loose muscle. The flanks are thick and saggy as well do after deflation (losing weight and liposuction) those will only be one saggier. So, you are in reality a great candidate for an extended or Circumferential (360) Tummy Tuck, thigh muscle repair and done liposuction in order to achieve a small waist and nice and tight abdomen and flanks. In my practice I look for a BMI approximating 30 to lessen the risks of complications during and after surgery and to enhance the results of the procedures. It is best to be within 10 pounds of your weight goal for three months prior to the BBL. If the waist was smaller at one time, the waist can likely be reduced with liposuction to those same measurements or smaller provided the muscular and facial integrity has remained the same. I recommend you to schedule a consultation with a board certified plastic surgeon.

It is not wise to determine the type of surgery you need before you lose the weight. The ideal is that once you have reached your goal (BMI of less than 30 at least), you are consulted again to determine if skin will be in a good condition to handle liposuction or if you need a skin resection and tightening surgery. However, at least in the abdomen, you are already a candidate for a tummy tuck. Hope this helps
