The problem is that your breasts have bottomed out so there is looseness of the lower skin if the breast. This can be corrected by cutting out a horizontal strip of skin from the lower part of the breast where you probably have a scar anyway

What you have is referred to as "bottoming out", where the nippleremains in good position, but gravity stretches the skin below the nipple. It is easily revised by reopening the line under your breasts and removing more skin and or breast tissue.
