Based on your photos you would be an excellent for revision breast surgery. It appears that you would benefit from a further reduction, breast lift and possible breast implant. The problem is that your breasts have bottomed out so there is looseness of the lower skin if the breast. This can be corrected by cutting out a horizontal strip of skin from the lower part of the breast where you probably have a scar anyway

What you have is referred to as "bottoming out", where the nippleremains in good position, but gravity stretches the skin below the nipple. It is easily revised by reopening the line under your breasts and removing more skin and or breast tissue.

Go back to your surgeon and discuss your concerns. The lower scar can be opened and reposition your breasts and remove excess tissue and skin. I would suggest going some smaller since your previously stretched and thinned tissue is not supporting this weight well. Your nipples are probably in good position and may not need to be disturbed. Your surgeon might give you a better cost than someone new.
