Septal perforation can be a challenging issue to fix. These septal holes can be fixed if they are a certain size and location. Larger holes and further back becomes more challenging. A rhinoplasty can address the "bulbous" part of your nose. If additional reinforcement of the tip structure is needed you may need to have cartilage from your rib harvested since the septum may not be in a good state to donate additional cartilage. If the main problem of the septal perforation is bleeding you may want to try some non-surgical options such as saline nasal spray, a room humidifier, or other methods of preventing the nasal lining from getting dry and irritated.Good luck with your surgery.Dr. Remus Repta
Hi,Based on the photos you provided, you appear to have a bulbous nasal tip with thick nasal skin. The bulbous tip can be reshaped and refined; and grafts may be required for tip support. In short, you can absolutely get improvement through Rhinoplasty. The septal perforation is a bigger challange but depends on how big it is. I recommend consulting with one or two Facial Plastic Surgeons who are double board certified rhinoplasty specialists - Facial Plastic Surgeons have training and experience specific to the face and nose - and with numerous before/after photos with results you like and seek. Bring in this picture of the nose you would like your nose to resemble. Make sure you ask for computer imaging so that the doctor can provide a good simulation of idea how your nose can realistically look like after surgery. Nasal swelling is a process. For this reason, patients should continue to follow up as post procedure care can help limit swelling and decrease chance of scar tissue from occuring. Noses need at least a year to heal.
Dear Samantha, It is not uncommon for patients to have spider veins on the nose and these can be treated. I would however suggest that you have the veins corrected after the rhinoplasty surgery. Best regards, Michael V. Elam, M.D.
