Eye bags only present when I smile. Would dermal fillers help minimize their appearance? 51 y/o (photos)

The eye area needs significant combinations to get improvement. Lasers like erbium, co2, halo, profractional/contour, or Fraxel can all be used to build collagen long term. To answer your question about lower lid bags when you smile. There are a few reasons for lower lid bags: fluid, fat, or fullness. An evaluation with your oculofacial plastic surgeon can help determine which is the most prominent in your individual situation. When the bags appear only when smiling and not at rest it is generally more of a lower lid fullness, sometimes called "jelly rolls" of the lower lid. This appearance is generally improved with botox injections to relax the overactive orbicularis oculi muscle of the lower lid that activates when smiling. The effect of botox helps soften the bags and squinting appearance and gives your eye a more open, "disney-princess" look. Every person is different and your surgeon can help determine the appropriate course of treatment for you.

I don't think that you have eye bags. The contraction of the obicularis muscle when you smile is creating the wrinkles and bulge. Some patients have obicularis hypertrophy which can be reduced surgically. in most cases, I recommend some botox in this area and either a chemical peel or laser resurfacing to give a smoother appearance to the skin.

We would need to evaluate you in person first. Cheek filler should be first injected before we decide to treat the under eyes.

Your "eyebags" seem to be a result of skin laxity and not a volume loss. I don`t think you are the best candidate for an under-eye filler (works best on young patients with hollows rather than laxity). I would better suggest looking into laser resurfacing or surgical options (although are more expensive and would probably give you better and more predictable results). What you have are not eye bags. What it is is that every person as they age loses volume in the face and since the face is slowly deflating it leaves a little bit of extra skin behind. Think of the face being a balloon that’s losing air. When you smile the cheeks simply rise and push on the extra skin in a lower eyelid creating that excess skin bulge. My approach to facial rejuvenation is with the use of the patients on the fat. Using patients fat can restore the volume that the patient had when they were much, much younger while removing the great majority of wrinkles and extra skin.
