Your nipple placement is good. I think you should get smaller implants and have more skin removed from the inframsmmary fold. There is too much skin in the lower breast

It is not unusual for patients to require a second breast lift. One important consideration is the type of breast lift. They are not all created equal. The vertical lift is most effective. I have learned to be aggressive in removing tissue from the lower pole to cinch it as tight as possible, producing the most perky appearance. This reduces the risk of a persistent lower pole bulge (plastic surgeons call this a "mastopexy-wrecking bulge"). Often when doing a secondary lift it is best to do it on both sides, even if one side is saggier, but that is a judgment call. Unfortunately, gravity and tissue elasticity work against us. The good news is that you can repeat the lift, especially a vertical lift, in the future.

Your results are typical of the vertical scar lift. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be placed if additional volume is desired.

Please repost photos without covering any part of your breasts and a closer view. You do have uneven shoulders and scoliosis which increases the appearance of being uneven when actually you may be more even.
