Would a malar osteotomy for flat cheekbones be a good alternative for cheek implants? What are the risks? (photos)

Hello I have flat cheekbone, I look tired, old and manly because of this. I will make HA filler to see if I like the results because it's not permanent. But in the long run I would like to do a malar osteotomy and not implants. What do you think about this operation? Is it risky compared to a Lefort 1? Are the results symmetrical? What are the biggest risk in this specific surgery? On the studies on the net it seems safe and predictbale. I would like to have your advice. Thanks
