Pseudo-sagging/jowls present when smiling? Options? (Photo)

Hi Ive had a 8mm genio, chin shaving,jaw implants, lots of sculptra ... yet I have the look of sagging skin when I smile. At rest my skin doesnt look or feel saggy. I dont have any extra skin that could be excised, I think. How can I treat this issue? It makes me appear much older when I smile and its troubling me. Thanks!
