Insulated needles allow targeting of the deeper dermis for tightening benefits. For more superficial complaints such as fine lines and shallow acne scars, non-insulated needles would be more beneficial. Although RF energy is safer in patients of color compared to lasers, any significant heat can result in increased risk of post-inflammatory hyperpigmentation. The RF (Radiofrequency) Microneedling devices may offer the option of use of non-insulated needles vs insulated needles. With the Vivace RF Microneedling device, the insulated needles only deliver energy along the bottom .4 mm of the needle, protecting the epidermis. This allows the procedure to be performed on darker skin which is at a higher risk for hyperpigmentation. The non-insulated needles deliver RF along the full length of the needles. They are used for lighter skin types.

An insulated machine has the base of the needle covered, with only the tip exposed. This allows all the energy to be sent to the tip only. The goal is to create micro-channels and to stimulate the dermal layer to make collagen. That is a valid question or concern. The Radiesse was most likely injected a level much deeper than 2.5 mm. Either sequence of Radiesse then micro needling/dermaroller OR microneedling/dermaroller then Radiesse is appropriate. You will require several sessions...
Not normally but if you have dry eyes and say a preexisting ectropion, the use of microneedling to tighten your eye area can lead to more problems. Micro needling itself will not worsen Sjogrens, but any trauma can cause Koebner, eg. in discoid lupus, vitiligo etc. Best if you see a Board...
