Wow, yes, your situation is challanging, and shows the severe consequences of going to a "general Ob/Gyn" for cosmetic/plastic work on the vulva (labia, vaginal opening.) Yes-- I have seen and worked on similar "catastrophies" in my 25 years practicing female genital plastic/cosmetic surgery, and my 10 years specializing in revisional work. You have a combination of "train tracks" and fistulae secondary to the wrong sutures, applied in the wrong way, and left in far too long. Whomever you see, make sure you know that he or she is a bone fide expert in revisions/reconstructions, and ask to see at least 10 B&A photos of their revisional work, plus at least 25+ photos of their primary work to get an idea of the techniques they utilize. Neither I, or any other revision expert can say with certainty the exact method they would use for your revision, as regardless of the quaity of photos provided (and yours are of excelent quality), things are always different "in-person" with a hands-on 3-dimensional evaluation. That said, and understanding that, with the damage done by your original surgeon, you never will have the idealized results possible with a primary procedure, a skilled revision surgeon can certainly make things much more aesthetic. This would most likely be done with a combined approach of a tight linear trim + RF "airbrushing" and perhaps combined with a mini-wedge for the "tunnels," and will resuly in a flattened, "Barbi-Style" look. Whichever, make sure that you see a revision surgeon who is savvy with ALL 3 of these techniques. I suggest that you do your "due diligence" and look at revision surgeons around the US and Europe, arrange virtual consultations, see their work, ask for their fees, etc. Unfortunately, "Insurance" will never cover this work (if the surgeon says (s)he will do it under Insurance, run the other way, as only non- genital plastic surgeons will accepth the super-low fee "insurance" pays...) This will cost you more than if a skilled genital plastic surgeon had done the work in the first place, and some surgeons charge rather obscene prices. Ask for specific "out-the-door" prices from the "experts" and see who is most cost-effective. Despite the complexity, this is work that can be performed under local anesthesia and in-office.
