Breast augmentation lift (mastopexy) problems.
These issues can occur for a number of reasons including weight loss, thin tissue and lack of support.
Your existing breast lift incision will be helpful to remove more skin and provide a stronger lift for your breasts. I definitely would use mesh for additional support internally. Areolas can be reduced, and the scars improved using better internal sutures. You should be able to achieve a very acceptable result with this revision.

Thank you for your question. Unfortunately, there is no easy fix. Many years ago, I pioneered a technique specifically for patients like you termed "Reductive Augmentation". This involves an internal bra-capsulorrhaphy, revision of the mastopexy-lift, and excision of the lower pole breast tissue. You should be able to have a beautiful appearing breast with this technique. Best wishes to you.

Your breasts have not been elevated and this is typical with the anchor pattern. The weight of your implants is stretching your lift and will continue to descend downward. You will need a revision. The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

One nipple is in the correct position and one is a little down. This can be fixed in the office. You might consider going down in size because you are starting to bottom out from the added weight. With your original sagging breasts, the tissue was stretched thin and will not support heavy implants. With your anatomy, you cannot have the round high breasts since naturally your breasts sit low on your chest. The bases of your breasts cannot be moved.
