Can I have a hematoma without bruising?

difference in size between one breast and the other. They are extremely painful on

You most certainly can have a hematoma without bruising.  Bruising is often a late sign.  Check with the plastic surgeon.  It is hard to tell from this photograph.

Your breast seem fine based on just seeing your picture. I would recommend that you see your surgeon for further evaluation. 

In the posted photo I see NO swelling which would indicate a hematoma. Best to see your surgeon ASAP. 

From your photos, you do not appear to have a hematoma.  It would be unusual to get a hematoma bilaterally.  Also, with an enlarging hematoma, there is usually quite a bit of pain and discomfort associated with it.  You likely have swelling that is waxing and waning with your activity.  Swelling usually takes at least three weeks to resolve for the most part.  You should consult with your surgeon to discuss your concerns.

It does not look like you have a hematoma.  Swelling can come and go especially with increased activity.  Please follow up with your PS.

It's doubtful you have a hematoma.   See your plastic surgeon to confirm.  You may need to limit your physical activity until you are healed.  Take care of yourself.
