I have chronic rhinitis and sinus issues. I seen several ENTs who have said I have a deviated septum. (I’m sure everyone does to an extent) I would like to make my nostrils more symmetrical I would like to minimize the curves of the upper part of the nostrils What kind of surgery do I need, what should specialities should I look for in a surgeon? Are the goal pics attainable?
Answer: The crooked appearance at your nasal base maybe due to a deviated nasal septum. Based on your narrative, the most important thing to do is to find a rhinoplasty specialist who understands how to deal with both functional and cosmetic nasal issues. You give a terrific example that separation of somebody's nasal function and their nasal appearance is largely impossible. One affects the other. The majority of patients that visit us have some combination of cosmetic and functional concerns and these are best addressed at the same surgery. Please consider meeting with several reputable rhinoplasty specialists for computer Imaging so you could see what to expect from both a functional and a cosmetic standpoint after surgery. Answer: Rhinoplasty You would benefit from a rhinoplasty and possible septoplasty. The medial crura, which is the cartilage of your nostril, is being blocked and is asymmetrical so this will have to be corrected. Answer: Asymmetry and treatment options The entire phase is fairly asymmetrical on all individuals. A lot of times nasal asymmetry is highly related to asymmetry of the rest of the face. Treating a symmetry is inherently difficult and often just substitutes one a symmetry for another. That said you could have your nostril, reduced and moved in slightly on your left side. The procedure can leave a visible scar. To get a quality assessment and better understanding of treatment options I suggest having multiple in person consultations with plastic surgeons in your community who have proven track record with Rhinoplasty surgery. Rhinoplasty surgery is fairly complex, and most plastic surgeons will agree that it is some of the more difficult procedures to perform well on a consistent basis. For that reason provider selection becomes very important. Being bored, certified, and plastic surgery with years of experience and overall good reputation does not mean somebody is good at rhinoplasty surgery. Ideally look for providers who either exclusively focus on rhinoplasty surgery, or those who have rhinoplasty surgery as a major part of their practice. Look for plastic surgeons, who have one or two decades of experience doing this procedure. Considering how much variation there is in skill and experience for rhinoplasty surgery I recommend patients have multiple consultations to help find the best provider. During each consultation, bring pictures of your face, taken the same way plastic surgeons take before and after pictures to use as a reference. We typically don’t remember what we look like very well. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patients who had similar characteristics to your own. And experience provider should be able to show you the before and after pictures of at least 50 previous patients Without difficulty. Being shown a handful of pre-selected images, representing only the best results of a providers career may be insufficient to get a clear understanding of what average results look like in the hands of each provider, what your results are likely to look like or how many of these procedures have actually done. Take careful notes during each consultation, especially regarding the quantity and quality of before and after pictures. If you’re going to have a septoplasty done in conjunction, then you should also inquire about that providers experience doing septoplasty surgery. The procedure can be done by plastic surgeons or facial plastic surgeons. Most facial plastic surgeons have an ENT background with anesthetic fellowship afterwards. General plastic surgeons typically have a background in general surgery followed by 2 to 3 years of plastic surgery training. in the end, the training isn’t as important as someone skill and experience. That said those with an ENT background may have a slight upper edge in regards to the management of airway obstruction due to septal problems. You are right that most people have some septal deviation. The human face is More asymmetric than most people realize. The human brain is accustomed to seeing facial asymmetry as being normal. That’s why we don’t see people as a symmetrical when they in fact are. In fact, looking at computer generated, perfectly symmetrical faces tend to have an odd and unfamiliar appearance.
