Thank you for your question. You are asking if pre-jowl implants can help marionette lines, or if it just adds volume to the jawline. You also indicate that you don’t want to use something more permanent like Sculptra. I can certainly give you some guidance in the absence of a physical exam. I’m Dr. Amiya Prasad. I’m a Board Certified Cosmetic Surgeon and Fellowship-trained Oculoplastic Surgeon. I’ve been in practice in Manhattan and Long Island for over 20 years. I am experienced in treating the jawline, jowls, marionette lines, nasolabial folds, and cheek volume using implants and injectable fillers. I use a technique of deeper facial filler placement called Structural Volumizing, or the Y Lift, which is not practiced by most doctors. To first answer your question, yes, pre-jowl implants can help improve the marionette lines as it improves volume along the jawline. However, pre-jowl implants alone will not be enough for best results of the marionette lines. I would add a hyaluronic acid filler directly underneath the mesolabial fold to improve the depth of the line, and perhaps in the chin, as well as fine-tuning along the jawline with prejowl implant. I do prefer to use thicker hyaluronic acid fillers like Juvederm Ultra Plus and Juvederm Voluma to correct facial volume in areas like the jawline and jowls, chin, and cheeks over Sculptra. Sculptra is more appropriate for global facial volume loss due to illness, and the effects of Sculptra are not seen right away as it works with the body’s reaction to the material poly-L-lactic acid to create volume by stimulating a collagen response within your body. This process takes time, and is not as predictable adding the exact volume and contour you may want to see. Hyaluronic acid fillers use the material alone to add volume, so results can be precisely customized by the doctor, especially when using the Structural Volumizing or Y Lift technique where filler is placed on a more solid, stable, foundational level atop bone, and between the muscle for stability and contour. This is opposed to traditional filler placement at the soft tissue level, where filler can migrate, and look soft and doughy. Thicker hyaluronic acid fillers last 1-2 years, and are reversible as they can be dissolved with the enzyme hyaluronidase. These thicker fillers can be administered using blunt cannulas instead of needles, and results are seen pretty much immediately, perhaps needing a day to settle and for a little swelling to subside. I would apply filler to balance the whole face, so it can include the cheeks, jawline, chin, and areas around the mouth. It’s important to note that we may notice specific areas of the face, but people look at our faces as a whole. I usually take a conservative approach in filler treatment, and see patients again in two weeks to see how the filler has settled, and if additional enhancement is needed. I suggest that you look for doctors who are experienced with deeper filler placement techniques, and also perform facial surgery so they know the facial anatomy well for the ideal areas where filler can give the face more structure and definition. I hope you found this information helpful. Thank you for your question.
