Why is there a double crease & why does my eyelid still feel different? (Photo)

He or she did not commit eyelid malpractice.  On the other hand, the surgeon did not help you as much as you needed.  You have upper eyelid ptosis.  The ptosis was present in the before pictures.  It is possible that the eyelid surgery made the ptosis a bit worse but it certainly was not addressed by your eyelid procedure.  Additionally, your surgeon did not make a single hard crease in your upper eyelid.  This maneuver is called an anchor blepharoplasty  and this is an exceedingly rare skill that is used to snug the skin and muscle of the upper eyelid platform like a bed sheet so you have a smooth upper eyelid platform.  This also supports the upper eyelid platform skin and orbicularis to the anterior levator aponeurosis to support the upper eyelid lashes (makes the lashes perky).  At the same time, this ensures a single uniform upper eyelid crease.  Ptosis surgery can also be incorporated into the work on the front of the eyelid.  Alternatively the upper eyelid can be lifted with a conjunctival muellerectomy performed on the back surface of the eyelid.  Improving your current eyelid result is what I do in my practice.  However, it will be challenging for you to find a local surgeon who even understands why you need revisional surgery.  Please understand that less than 1% of the eyelid surgeon can accomplish what you need to revise your upper eyelid procedure.  If you do travel to a larger city for revisional work, I strongly encourage you to get more than one opinion.  It is burdensome by essential.  If you are not surge of the options being recommended, consider reposting to Realself for more feedback.

I disagree with the previous expert poster. I think your result is very acceptable as compared to the before posted photo. Therefore I would recommend against any additional surgical intervention at this time. The question is do you appear better tha=n before the surgery? My view is yes...
