I had silicone injections put in once 9 yrs ago in my butt. I have had no infections complications or pain. I just don't want to wait to get them removed sometime in the future. I want to get them out now. However is it possible if one does not have a lot of the product in their butt to get a fat transfer the same day?

I don’t suggest transferring fat to the butt for reconstruction on thesame day as silicone removal. The surgeries should be staged. I would suggest youwait two to three months for the swelling to go down after silicone removal. Theseprocedures are not easy. Please seek an experienced, board-certified plastic surgeonwho has done many of these silicone removal procedures. Multiple stages isthe way the to go for the treatment of your buttock. I perform hundreds of silicone removal procedures each year, and I do strongly believe that reconstruction should come after removal- not at the same time. This ultimatley gives you the best chance for a good cosmetic outcome. The harvested fat will not survive the transfer when the area has a compromised blood supply. Additionally, the product removed during surgery can very easily contaminate surrounding tissues and that's not something that bodes well for newly transferred tissue. Bellafill and Artefill are known to cause granulomas and permanent nodules with swelling. This is not the case for everyone. Silicone biopolymers can also cause similar chronic inflammatory conditions. In my practice, I specialize in the removal of Bellafill and Artefill injections, in addition to silicone biopolymers, from the face and body. Initially, treatments are non-surgical with corticosteroids and immune modulators. If unable to be corrected, we can also perform surgery in some cases. Silicone biopolymers can also be removed from the body if there are signs of migration or inflammation. All of these filler-types should be removed as early as possible. Lastly, our office is conducting ongoing studies of plasma-assisted corticosteroid application, which allows for diffusion of the treatment medication in superficial tissue without causing atrophy. To answer your question, no, do not attempt fat grafting at the same time as removal of the silicone. There are lots of reasons for this. However, if you are not having any problems from the injected silicone, then leave well enough alone. Assuming these injections were done some years in the past, then if you have no problems now, you likely will not in the future. Surgical removal of the injected silicone will result in significant deformities which will be difficult, if not impossible, to totally correct, even with fat grafting.
