PS did a 2mm punch on small ice pick. Stitches out after 7 days but I was left with a hole. A month later, he reco'd a re-punch. Said that he wouldn't have to make the hole any bigger;just take out the 'core' again. Now it's still indented and looks bigger than 2mm. Tried Esthelis filler 5x (last time in March) but it looked terrible; donut effect though it's not tethered. Also did 1x frac CO2. Re-excise? Dermasanding/dermabrasion? Filler again? Help!! It's seriously killing my confidence.

It is hard to give exact advice. I don't like to do the punch excision, it always leaves a scar. I would try lasers, chemical peels or Infini for this. You must also get on a good topical regime including Retin A or Retinol for exfoliation.
Punch excision is an option. In my opinion, not the best one. Grafting could work but there are other ways to make it better. There are ways to get these scars to raise up with lasering or peels. TCA deep peels can help raise the up. This is often called TCA CROSS using 80% or stronger TCA. B...
This appears to be a normal healing process. I suggest using normal saline instead of hydrogen peroxide as this damages the surrounding skin and can actually slow down the wound healing process. Please consult with an expert if you experience further concerns. Best, Dr. Emer
