How do I treat a worsened chicken pox scar after punch excision?

That scar shouldn't have been punched out, anyone who does a significant amount of scar surgery know the scar would be 2-3 times the size after in a straight line.  With that said at this point you need lasers, subcision and fillers to improve it.  I suggest you see a laser and scarring expert.  Best, Dr. Emer.

Hi K. We likely would have suggested laser resurfacing before any excision and this is what we would suggest now.  Fractional laser resurfacing works best on new scars and can help soften the current edges you have on the scar.  If it does not result in enough improvement for you, adding a little filler later on may be an option as well.  To see scars treated with fractional resurfacing in our Los Angeles County office, click on the before and after pictures link below.  

Hello 'kirakat', thanks for your question.  There are several options for scar therapy.  The initial management up to a full year is massage the scar, avoid sun exposure, biocorneum cream, chemical peels, and occasionally steroid injections if the scar is thick or raised.  The next option is CO2 laser skin resurfacing which can help with smoothing out the contour of the surface.  If those still do not work adequately, then formal scar excision with closure would be recommended.  This will lead to a longer straight scar, but the goal would be to close it along a relaxed tension (Langers) line so that it heals flatter and smoother.  This would be perpendicular to the orientation that it was closed in my opinion.  Always consult a board-certified plastic surgeon and let me know if any questions.  Good luck!-Dr.92660
