The lower side could have internal tightening to raise the inferior pocket. This could be done at the same time as the areolar scarring and size is redone.

It really depends on what you're going for. If you want more volume in your lower pole then a capsulotomy below the nipples on both sides would allow the lower poles to expand. If you're saying you would like more of a crease and a curve back to the chest wall like a natural breast (less of a stuck on appearance), then that's a different technique, and I would use the Original Internal Bra, my strong permanent internal suturing technique, to provide more of a fold definition with a little overhang.
