I am 26 and have under eye bags from fat pads under my eyes, I was told by one surgeon that I am too young to get a lower and I should have under eye filler instead, however I also get a lot of water retention and puffiness and heard this will make it worse, is this the answer I’m going to get from all oculoplastic surgeons? Is there anything I can do to help reduce the size of the fat pads like eye exercises or microneedling etc?

Answer: Eyelid Surgery Hello! Thank you for your question.As far as I can see from the photo you do not have any extra skin under your eyes. The lower eyelid blepharoplasty is give you the look that you want. For the hollowness I recommend you fat injection to under eye area. Also your eye corners are downward. With the cat eye surgery your eyes would be much slanted and upward. If you can provide photos with better lighting, from the side of your face and 1 meter away I would be able to make a proper evaluation.Best Regards!

In many cases this is because each surgeon has his/her our own comfort level with filler, fat pad surgery and/or fat transfer. While each of these may be options in your case, you will likely get different recommendations from different providers. It's hard to tell from the photo if we would suggest a lower lid surgery where correction of the protruding fat pad is undertaken or rather a camouflage of the area using Restylane dermal filler (see link below). Given your age, we would likely be leaning toward filler at this point to see if that could help before any surgery.

Under eye fillers rarely improve the appearance of lower eyelids and complications such as lumps bumps gray discoloration and even blindness have been reported. Looking at your photos carefully I do see a small shadow or dark line underneath the lower eyelid. This is caused by an attachment of the lower eyelid to the bone underneath the lower eyelidf fat pockets. This can be addressed with a transconjunctival lower eyelid Blepharoplasty with Arcvus Marginalis release and fat grafting.
