From the photo it appears you would benefit from a full tummy tuck or direct excision of this area. An examination is needed to know for sure. I encourage you to visit with a board certified plastic surgeon near you for an evaluation to determine what is the best recommendation for you.

Hi Heather1992,Looking at the image, I suggest you consult with a surgeon as soon as possible for a quick check up with a full body can because I do not want that to get worse for you as I am looking at it, it can continue flapping down and growing larger if not taken precautions. It seems like an incision needs to be made with added liposuction. Scars may show after surgery but that is better than this leading to something worse down the line. Unfortunately, many times, the weight of the breasts push fat down and create a lump sack. This is excess fat over the lower ribs.

The lax tissue creating the rolls would require excision to treat, leaving you with scars across the abdomen.
Seek the opinion of a recognised plastic surgeon who can assess you properly and advise treatment.

The role under your breast is a skin fold that happens because of skin attachments at the base of the skin fold. The same skin attachments go around the chest to the backside and create the fold on the back sometimes called the back bra fold or roll. Because the problem is skin laxity with a skin fold it is best treated with a skin procedure. It can be released during a full tummy tuck if the separation a fat and skin from the muscle layer is done high enough up on the chest wall. It can also be done through a reverse tummy tuck. Treating this as a fat problem with Liposuction will give a limited results that will most likely be unsatisfactory. It is primarily a skin laxity issue.
I suggest having a few in person consultations for the local board-certified plastic surgeons. Avoid providers who claim to be able to fix this with liposuction. If they do ask them to show before and after pictures of other weight loss patients who have a similar condition. Massive weight loss comes with skin laxity and this is a good example.

The best way to manage tis situation is to actually remove this. A horizontal incision under the breaston both the side will pull up the excess skin & remove this. You will have a scar under the breast but most of this extra skin can be removed.
