Tear trough filler at 19? (Photo)

Question

Hello wanted some opinion on getting tear trough filler or another procedure to reduce darkness under my eyes. I am 19 and have a relatively healthy diet/ sleep 7-8 hours a night but still have some darkness under my eyes. I’ve use a vitamin C brigtning eye cream for around 2 years but the darkness under my eyes still bother me. I have included 2 pictures one with flash and one without. Any opinions/advice would be greatly appreciated!

Thank you for your question and photos. You may benefit from dermal filler injections to treat the under-eye darkness you’re referring to. These hyaluronic acid formulas are used to fill in hollow areas and create a smooth transition between the lower eyelid and the upper cheek area. While filler treatments are not as invasive as surgery, they still require a great deal of skill and precision to achieve the best possible results. In my office, we use a micro-cannula to inject this area, which minimizes bruising and swelling and ensures the smoothest outcome.

At home peels like cosmelan, enlighten and aerify along with dermarolling (MTS roller with anteage MD ampules) should be started now. There are many things you can do to treat under eye hollows and dark circles. A surgical option to remove skin and/or fat will such as lower eyelid blepharoplasty with or without fat pad repositioning will be the best long term. There are many things you can do to treat under eye hollows and dark circles without surgery. Non invasive less downtime options such as fillers (Belotero, Juvederm Volbella​), lasers (Moxi/Halo/Fraxel/CO2​), Renuvion (J-Plasma), microneedling/PRP, stem cells and microneedling RF like Fractora PiXel8 or Morpheus can all help give some improvement. Accutite can often be used or stronger lasers like erbium or co2 spot treatment can tighten the skin and give very quick improvement in wrinkles and texture. At least from the photos provided there appears to be some volume deficit in the upper cheek region, Since the upper cheek region is a major structural support for the tear trough area, volume deficit there as evidenced by some flattening of the upper outer regions of the cheek in the photos, can be responsible for the development of tear troughs, hollows and bags. So, very often, the use of an appropriately robust HA filler injected into the upper cheek region, rather than direct treatment of the tear troughs, can not only refine the contour of the upper cheek, but create a smooth transition between the upper cheek and under-eye area WITHOUT THE NEED TO DIRECTLY INJECT THE TEAR TROUGHS. You would be wise to make certain to consult an experienced, board certified cosmetic dermatology injector.. Hope this helps.

An in person evaluation is always best, but it appears from your photos you may be a candidate for tear trough filler.  This should help improve any volume deficit which in turn may alleviate some of the shadowing that concerns you. The photographs you submitted do not unfortunately provide enough information with respect to the problem you are seeing. There could be two issues with you. Number one, you have flat cheeks, making it look like you have bags in the lower eyelid area or you have real lower eyelid bags. If you have lower eyelid bags, the procedure is quite easy. Transconjuctival blepharoplasty will take care of it. On the other hand, if your cheeks are flat, it is my opinion that fat grafting is the single best option, which would result in natural and permanent appearance around the area and rid you of the shadow you are seeing.
