Hi, I’ve recently noticed the area where my right ear joins my face the skin is loose and has created a crease line - can this be helped & what may have caused it please? 43 yr old female with T1D, EDS type 3

Aging is a common factor in the development of loose skin on the face and near the ears. Trauma may also be a factor. The excess skin can be removed.

It is possible to separate the ear from the face by excising the tissue and closing it.  It is difficult to create symmetry by doing just one ear.  Unfortunately, you have not provided a photograph of the other ear.
