I have a fat nose with a slight hump that doesnt bother me. I was to address 2 issues with surgery: - my nose is fat i want to reduce its size - when i smile my nasal tip goes down and the nose looks even wider - it has an irregular shape What procedures would you advise to solve these issues? (Please specify if open or closed is more suitable) Please note i want extremely natural results not a completely different nose

The procedure would involve small, dorsal hump reduction, would take down the dorsum of your nose, as well as tip, refinement to improve projection, and decrease the boxiness of your nose as well as correct your tension tip when smiling. Additionally, I think you would benefit nicely from a small weir base resection, which would narrow your base. these procedures combined will create a very natural nose that fits your face.

Answer: Typical Hi hirakhan. It looks like you would be interested in the "typical" Asian rhinoplasty procedure in that you have similar concerns and goals as what a patient with a typical Asian nose would have. In general, there is less cartilage in the nose of an Asian patient, so this is why there is less support to the tip. In terms of what to do, if you consulted with me, I would discuss shaving the hump, doing Weir excisions to narrow the alar base, doing tip modifications to get the tip less bulbous and more narrow, and adding a septal extension graft to support the tip and give you a little more structure. You should also consider osteotomies, although you did not specifically specify that you think the whole nose is wide. A big limitation you should have is that Asians generally have thick skin (and you do based on your photos), so that will limit how small your nose can get from surgery alone. Good luck!

Answer: Rhinoplasty You need your jump reduced and the bridge narrowed. It can be done either open or closed. Many surgeons don’t know how to do closed so you will probably get more recommendations for open, which would work also
