I had a septo rhinoplasty last summer (July 2021). I had a deviated septum to the right. Over 1 year on I have a hard bump on the right hand side of my nose, making my nose look asymmetrical. This bump effects my breathing as it’s blocking part of my nostial internally. Do I need a second rhinoplasty or can this be fixed another way? If I need a revision should my surgeon be responsible or should I go elsewhere?

You should return to your original surgeon. Cosmetically your result is great. Improving your breathing may involved something very simple and not necessarily surgery

A hard bump on the outside of your nose may not completely explain the difficulty you have breathing.  You would need a full assessment of the nasal anatomy that can affect the function of the nasal airway.  Externally, they would assess the nostrils and whether they are narrowed or collapse with breathing.  Internally, they would assess whether septum, position of the cartilage, and/or turbinates are restricting the airflow.  Depending on the cause, you may need secondary rhinoplasty.  I would recommend discussing your concerns with your surgeon.  They would know what techniques were performed in the operating room and would know your current anatomy the best.

It's difficult to give you a definite answer in this situation without an examination. The picture you posted doesn't really give enough detail to make a great assessment. A bump blocking your breathing is certainly unusual and warrants further evaluation. It could be a number of things: scar tissue, cartilage, or bone. It's possible that the septum re-deviated and resulted in this situation. Regardless, see your rhinoplasty surgeon first and they know your nose and what was done better than anyone. Make sure they understand your concerns and can explain to the best of their abilities what it is that you're experiencing and potential treatment options which may include revision surgery.
