What can be done to treat my perioral mounds? (Photo)

I have perioral mounds. I have a big face already but when i smile those perioral mounds even get more prominent. Those mounds makes my lower face even more heavy and gives me droppy moth corners. I could not find any doctor in London who remove/ help me with this. And i know it is not buccal fat removal. If any one out there could help please.

We can try lipo and also fat transfer to the other side of the knee. I personally treated patients with knock knees with liposculpture with very satisfying results.
