Mons pubis too high after tummy tuck - advice? (photos)

You are absolutely correct. The scar is too high and it’s pulling on your mons. Unfortunately there is no excess skin for us to pull down and lower the scar. This means that there is nothing we can do unless we use a tissue expander before doing something kind of like a mini tt. Tissue expanders are more commonly used for breast reconstruction, but I believe it would be useful in your case.

The tissue expander is placed beneath the skin and fat. It is expanded with saline ever so often so it forces the skin to grow. Eventually you should have enough skin above the scar. Once we have enough skin between the scar and the navel, it is pulled down towards the mons. Skin is then removed only from the mons area. So this would similar TA a mini tummy tuck but with the use of a tissue expander.

You may have had not enough skin for a traditional tummy tuck that keeps the scar about 7-8 cv above the top edge of the vulvar commissure. The two options then are to move the scar higher or to create a midline vertical scar which does have a higher healing problem but is what I prefer to do since then all the scar is hidden by a bathing suit except 1-2 inches coming straight up the middle which is not much of a cosmetic concern. It is unlikely that you have enough laxity to move your scar lower. Good Luck!

for you better to plan a revision tummy tuck ( also will be as scar revision with removal of old incision and will be again one lline) . In this operation you pointed also the navel but it will be well planned together before operation and /or on a video call more detailed to explain the choices. Finally to take the mons better and expected position with hiding scar under bikini line.

Thank you for sharing your question and photographs. Unfortunately nothing replaces an in-person examination but it does not appear that you have sufficient skin laxity to be able to revise your scar and have it placed lower on your abdomen. Seek out in-person consultations to get the best advice.

Your scar could be lowered and help lower your mons pubis as well, but it would involve two surgeries. A tissue expander could be placed above your scar to stretch out your skin. Once stretched, the excess skin could be used to lower your scar in the center. This would not be fun.

I'm sorry to hear of the issues you are having with your scar position and mons area. An in person evaluation may bring about a different opinion, but based on the photos it doesn't appear you have much excess skin. This would be needed in order to lower the scar and give relief to the mons area.

What time to be critical of plastic surgeons is during the consultation process. In my opinion, tummy tuck results should be based on plastic surgeons to get the most natural looking belly buttons with a very low side scar that follows an atomic contours naturally and evenly on both sides, leaving the torso, looking, attractive, appropriate and proportional from all angles, including from behind.
A full tummy tuck is very much a permanent and irreversible procedure. You no longer have the skin laxity to lower the incision in any significant meaningful way. Doing so would require adding an additional scar closing the opening of where your bellybutton now comes through the abdominal skin layer.
While it may be possible to lower the score, a small amount doing so in any significant weight, would require redoing the entire procedure(minus muscle tightening) and as mentioned earlier, would add an additional small vertical scar from where your current bellybutton incision is.
A question if we doing the procedure is going to be worth it in the end. Adding the additional scar which will be in a visible area, may counter the potential advantage of lowering the primary incision. It’s possible to lower the incision slightly without repositioning the belly button, but this gives less skin to recruit and would significantly. reduce how much the scar can be lowered.
Once a tummy tuck has been performed. It is unlikely patients are going to get a significant improvement from revision surgery.
The surgery needs to be designed correctly from the beginning.
