Want to go bigger after uplift with implant. Do they have to cut the nipple again? (photo)

I had an uplift implant already and I want to go bigger, I had lolly pop uplift. I’d I want another surgery do they have to cut the nipple again? I’m very worried as it’s a horrible recovery and it discolours your nipple and changes the shape… how can this be avoided please
