Hi, thanks for your question. Sorry to hear that you have had problems after previous surgery. If the indentation is related to scarring of the breast onto the muscle, then this will require release, and the muscle laid back down onto the chest wall. Some fat transfer may be an option of you have an adequate volume of fat available for transfer. You may require a staged approach (more than one operation) to achieve the best final result. A face to face assessment would enable more accurate advice to be provided.

You will need additional volume to round the breast out which will require fat transfers. You may also need release of scar tissue, if it is tacking it down to the underlying fascia or rib.

it is hard to tell for sure without an examination. Based on your photos, it looks like fat transfer would improve the shape of your breast. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

You need an examination to see what possibility could be done for your muscle. You don't appear to have much fat. But if you do have extra fat, a fat transfer could be helpful.

The muscle behind your breast is not properly attached to then chest wall.
It need to be sutured back and may need some fat grafting

In order to get you a quality assessment we need much better information.
Did you have a capsular contractions? What’s the capsule removed at the time of explantation? Why were your implants removed?
Most likely, the deformity is caused by your capsule or capsular contraction. If a capsulectomy was not performed, then starting by removing the capsule would be the way to go.
If your implants were replaced behind the pectoralis muscle, then your capsule is attached to the pectoralis muscle.
What does your surgeon who removed the implants say about your outcome?
If you want a proper second opinion, then you need to come prepared, bringing with you a complete copy of your previous medical records. This should include all before, and after pictures of both breast operations, the operative reports and indication for your second procedure.
It would also be helpful to read the notes of what your surgeon thinks is the underlying ideology.
Fat transfer is definitely not a good option because your problem is not based on lacking breast volume.
Operations need to be focused at treating the underlying problem and understanding the problem is the key to a successful outcome from any surgical procedure.
I suggest consulting with plastic surgeons who have extensive breast augmentation surgery experience.
Look for providers who have breast augmentation as one of their major operations and I’ve been doing this for at least a decade or more.
Depending on what underlying ideology is this may or may not be fairly straightforward or it may be quite difficult to improve on.
My best guess is removing your capsule will straighten out your breast fairly well.
