Will I be able to achieve more volume without complications adding fat around my 7 year old implants? And will it leave scar? Am I a good candidate for fat transfer around my implants? In my experience, fat transfer to breasts can add to areas over implants nicely, provided you have adequate fat to harvest via liposuction. Most of the fat stays, have to be cautious not to damage the implants. An exam and consultation with a plastic surgeon who performs these procedures is recommended to confirm you are a candidate as well as discuss your options and expectations.

Answer: Revision In your case, your implants are bottoming out and the implants are wider than your chest wall. They will continue to descend and fat transfer will not be able to compensate for the abnormalities you are developing. I recommend capsule plication. At that time, the implants are removed, the capsules are reinforced inferiorly and laterally and dissected more centrally to reposition the implants. At the same time, fat transfers can be done. Answer: Fat grafting needs fertile soil Although your photos are 'great', they tell us nothing about tissue thickness, one of many important factors to consider. Fat grafting data has shown safety since the early 2000's. Donor sites are important. I feel that scouring all four extremities and the torso to get enough fat is a bad idea. Instead a good candidate for fat grafting is the same person who is a good candidate for liposuction: a definitive area(s) to be treated to affect a positive change to the contour in that area(s). Unlike fat grafting the buttocks, where it is imperative to avoid placing grafts into the gluteus muscle, the use of high resolution ultrasound is unnecessary; avoiding placing fat into the implant pocket (or penetrating the implant) only requires tactile feedback. It is likely that your upper pole will have sufficient tissue to harbor fat grafts, but your lateral breast may be deficient and require multiple small fat grafting procedures to develop enough tissue thickness for your satisfaction. Bottom line is that you need to be examined first.
