20 f I got a lip lift over a year ago made the mistake of doing a at home 70% tca peel on the scar 6 months ago and the scar is stiff and raised should I get a revision lip lift

if it's stiff/raised, it's likely hypertrophic. the easiest thing to try on this would be steroid injections. I would work on trying to improve the scar first as best as possible but worst case, yes you may needc a scar excision/revision.

Not incompatible to each other; either:
-you were not at a candidate for an indirect lip lift aka "bullhorn lift) (do not confuse with the direct lip lift) for having short distance between upper lip and nose
-your surgeon miscalculated and over resected the skin at the nasal base

Or both; in any case, there is an evident over tension at the closure that is not due to genetical factors, it is a dehiscence due to mechanical strain, and this has bad solution; it'd be raising a pair of nasolabial flaps to restore the missing portion at the nasal base, not even skin grafting would be acceptable due to its poor aesthetic outcome for this specific purpose, it has to be nasolabial flaps.

Stay far from chemical peels, lasers, injections, etc, that is all wishful (surgical) thinking.

From your photo and history, the scar appears to be hypertrophic (raised and firm), likely worsened by the strong TCA peel you tried at home. A full revision lip lift may not always be the first step. Many surgeons would try less invasive options first, such as steroid injections, laser treatments, or microneedling, to soften and flatten the scar. If the scar remains stiff and raised after these measures, then a revision lip lift could be considered.

To flatten an unwanted elevated, thick, bumpy scar, the treatment of choice for decades has been the use of intralesional anti-inflammatory injections. Surgery would likely leave just another scar that likely would have to be dealt with. You would be wise to consult with an experienced, board certified cosmetic dermatologist scar treatment expert.
