How can I get his nose through surgery?

The nose in the first pictures has less volume in the tip with less width and flare of the ala and nostrils in the after photo. These changes can be made surgically during a rhinoplasty with alarplasty/alar base reduction.

In the first photograph, the skin appears thinner, the cartilage is more defined and the base of the nose is wide.  Your nose skin appears thicker and wider.  You will need a rhinoplasty to correct this.
