I have a bulbous nose that does not have structure and droops down. I have rhinoplasty surgery scheduled for January 2024, however I am having a hard time explaining what I want in my consultation. My main concerns are that I do not want an over projected tip. My tip already sticks out a lot from my face. I just want to make the tip less boxy and bulbous. I would also like to slightly lift the tip to correct the droop. What would be the specific terms I could use to explain to my surgeon? Thanks

Answer: How would I go about correcting a bulbous tip? From the side profile photographs, you have an overly projecting nose, and an under projecting chin. The under projection of your chin makes your nose look bigger than it really is. Consider a closed rhinoplasty, along with a chin implant for better facial balance and proportions. A closed rhinoplasty can accomplish shaving down the dorsal hump, decreasing the projection of the nose, and refinement of the bulbous nasal tip with all of the incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. Placement of a chin implant can be performed at the same time as the rhinoplasty, or separately under local anesthesia. In our practice, we place the implant through a small incision underneath the chin. Choose your rhinoplasty specialist wisely based on extensive experience, since rhinoplasty is the most difficult procedure to perform correctly in the entire field of cosmetic surgery.

Answer: Communicating with plastic surgeon First and foremost would be confirming that you met with the plastic surgeon through an in person consultation. If you haven’t done, so, then I suggest going back to the drawing board and scheduling it in person consultation. Not only do you need to make sure that the surgeon understands what you’re hoping to achieve but you need to make sure that the surgeon you’re selecting can perform at that procedure. Rhinoplasty surgery is fairly complex stuff and patients should be very selective when choosing providers making sure they have a proven track record with rhinoplasty surgery having done lots of similar cases in the past. Before choosing a provider, I generally suggest having multiple in person consultation, so you have some reference and comparison. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patients who had similar nasal characteristics to your own. An experienced plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. A highly experienced plastic surgeon shit in fact have hundreds or preferably thousands of before and after pictures to choose from. They should be able to find examples of previous patients who had similar characteristics and explain to you how they were corrected, and what the outcomes look like. You can then share feedback with the surgeon, telling them which results you like and why. The type of rhinoplasty you need is complex and will most likely require Cartlidge grafting to give support to the tip of your nose. Being board-certified and plastic surgery with years of experience, and an overall good reputation does not mean somebody has mastered every procedure, especially not complex procedures like rhinoplasty surgery. If you’ve already gone through the vetting process of selecting surgeons, and you have met with several surgeons during in person, consultations, as simply still have questions about the outcome of your procedure then I suggest you schedule another meeting before your upcoming operation.
