My chin is my biggest insecurity. When I’m not smiling nothing seems wrong, but as soon as a smile a toothy grin my chin sticks out far horizontally. I think it has part to do with my bone structure but more so to do with the fat on my chin pad. When I smile it seems to displace the flat onto the tip of my chin. What operations will give my chin a more subtle look. A chin pad reduction, a full out chin reduction, possible dermal fillers between the chin and lip to make it less protruding?

As backward sliding genioplasty or orthognathic surgery are not the correct solutions for a horizontally protruding chin. A submental chin reduction approach is best which can reduce the bone and soft tissue chin pad while not increasing submental fullness.
