Chin reduction surgery can be accomplished by shaving the bone or removing a central "wafer" of bone and re-positioning the chin in a posterior direction. Sometimes both are performed at the same time. The result should give you a well contoured, smooth jawline from back to front. In some cases, the soft tissue pad of the chin needs to be reduced if it is excessive. The surgery can be accomplished in the office under intravenous sedation. Good luck
Susan, Reducing a chin is a lot less common than making one larger, but there are a few of us that do a lot of these procedures. The bone needs to carefully removed in a way that preserves symmetry and protects the delicate nerves in that area. If you remove too much bone you will end up with excess skin, so there is limit to how much the chin can be changed. That said, you will be able to remove enough bone safely that your chin should look like a nice average chin. Your chin appears vertically long with an excessive soft tissue chin pad. As it is likely both bone and soft tissue need to be reduced, a submental approach to your chin reduction is needed. Given the 'magnitude' of the problem, it is likely that it may require two surgeries to adequately reduce the tissues and keep the submkental scar limited.
