Thank you for your question. The capsule is in very close proximity, (very close to), the implant. Most Board Certified Plastic Surgeons would remove the implant, and then do the biopsies that may be indicated. They would then replace the implant into the pocket. This is typically not done under local anesthesia. Please do talk to your Board Certified Plastic Surgeon. They will go through the process with you, and together you will make a plan. Jeffrey J. If a caspule biopsy is performed there is a high risk to the integrity of the implant. This risk obviously goes up as the number of biopsies taken increases. Another factor to consider is the capsule itself. If the capsule is thin this would be even more difficult than if it is thickened. Is it possible? Potentially yes. However there are risks to be considered.
It can be done, however, the implants will need to be removed before doing the biopsies for safety to the implants. If you want your implants, then they can be replaced. It will be easier on you to do this under general anesthesia or IV sedation. Not all surgeons will agree to do this under local anesthesia. A radiologist could possibly do a biopsy guided by Xray unless the capsules are very thin.
