About a year ago, I started to develop really bad undereye bags and dark circles and consulted with a surgeon. He confirmed my fear that the bags were probably caused by filler that I've had over the years. He didn't think attempts at dissolving the filler would be successful and told me I would just have to wait until they dissolved on their own. The last time I had filler was almost 3 years ago and the bags only seem to be getting worse. Are there any procedures that might help?
Answer: You received bad information. These fillers persist for years. I have seen patients with residual filler 12 years after their original service. As the product ages, it hold a disproportionate volume of water which is why these ancient services make things look puff. You need post septal hyaluronidase in a sufficient dose to remove your ancient filler. The treatment is straight forward if you understand enzyme dosing and orbital anatomy. That means that your dermatologist or general plastic surgeon should not to this enzyme treatment for you. I see many patients from out of town for this type of treatment. Treatment is very comfortable. I am just sorry you had to live with a bad treatment for so long. Response to enzyme occurs almost immediately and the full effect of the enzyme service is seen at 24-48 hours.

Answer: Improving Under Eye / Tear Trough with Injectable Fillers (Belotero, Volbella, Restylane Silk) -- add Lasers, Microneedling/PRP Fillers like Belotero, Volbella, and Restylane Silk are good for improving under the eyes, but keep in mind that a combination approach will give the most improvement. My patients try to get non-surgical treatments to avoid surgery, so we use fillers, lasers, microneedling/PRP and stem cells. Answer: Filler-Induced Infraorbital Lymphedema ( "Puffy Pillows" ) Difficult, Bot Not Impossible To Treat The problem described here follows a typical course for infraorbital lymphedema--i.e. lymph accumulation under the eyes that may follow an inappropriate placement of the filler or improper choice of too robust a filler into the thin, delicate tissues in this region. The history of persistence or occurrence years after the filler treatment is typical of this condition in my experience. If this is indeed lymphedema, lymphatic obstruction, there is little likelihood at this stage of this spontaneously resolving. In such cases, I have found that several rounds of and algorithm of intralesional steroid injections, 5-FU and hyaluronidase in varying concentrations and amounts to be helpful in shrinking these sacs. Make sure that you consult with a board certified cosmetic dermatologist with experience and expertise in treating this frustrating problem. Best of luck.

Answer: Recommendation It seems that there are some probs. First of all, as you mentioned, you look tired because of bulging undereyes and dark circles. In that case, we recommend Transconjunctival Fat Reposition + fat graft for undereyes. Another problem is pigmentation. It can be solved with Laser Toning which can help your undereyes brighter a bit. If you got filler before then better to disslove it first then see how it works then do the surgery.
