It is impossible to tell by this photo if there is excess glandular tissue or just fat. The treatments are almost always the same, however. Liposuction is performed and in most cases part of the gland is excised. If the gland is not excised and it is a contributing factor you will be left with puffy nipples. Do see someone experienced.
You would need a physical exam to determine if you have gynecomastia, in any case you do not look like you are very much over weight. You could lose weight and see if your breasts reduce thereby meaning you only have pseudogynecomastia, which means breasts caused by fat only. Be sure to see a board certified plastic surgeon who specializes in gynecomastia. Good luck!

From the photo it is really hard to tell if you have true gynecomastia. You do have some extra belly fat but you don't look very overweight. You should get examined by your primary care doctor or see a board-certified plastic surgeon to assess if you have gynecomastia and if surgery would be helpful. From the photo it does not appear like you have a significant case but exam is required to know for sure.
Based on the one photo you don't appear to be significantly overweight. Diagnosis of gynecomastia includes your history of the problem, visual appearance and physical exam. Using the photo supplied as a reference, you don't seem to have a significant amount of gynecomastia.Your best option would be to schedule a consultation with a board certified plastic surgeon with experience treating gynecomastia.

Sorry for your problem but I have treated many patients over the years with gynecomastia and gotten insurance approval for those that qualified. So go on some consults with experts who accept insurance and begin the approval process Dr Corbin
