Why can't I find a doctor that will do saline injections for fat atrophy?

Kenalog 40, or triamcinolone acetonide 40mg/cc, is a concentrated steroid that may result in dermal atrophy and indentations. Saline injections should be totally safe but temporary. I have heard the advice to treat steroid related atrophy with saline injections but have not seen much success with this technique. I think the difficulty with finding someone to do this for you is that there is not much in the way of data to support it. However, it should be safe as the saline is merely reabsorbed following the injection. You can also consider dermal fillers to temporarily fill in the areas of atrophy. I can't see how saline injections would help your atrophy. Your best choice is to use a dermal filler. A saline injection would only help for a very short period of time such as a few hours .I hope this information helps.

Fat atrophy from high concentration steroid injection (40mg / cc kenalog) usually resolve after 3-4 months. Should it persist more than 6 months, you may consider temporary or permanent filler, or even fat transfer. Saline injection is not commonly practiced as it gives unpredictable results.
