It looks like a hypertrophic scar, not a keloid, and seems to be the only scar that healed that way.
I would just excise it and see how it behaved in the healing process over several months, and possibly treat it with scar laser or steroid injection if it looked to be developing a raised surface again. Steroids are a double-edged sword and can backfire, leaving a white, atrophic, depressed area. Have it examined in person by a board-certified plastic surgeon.

it is hard to tell for sure without an examination. Based on your photo, it looks like this can be removed under local anesthesia. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

Your surgeon is responsible for the outcome of your procedure. I do not recommend people travel long distances for elective surgical procedures. The importance of having multiple in person consultations, the number of patients who have real concerns after surgery, the complications actually happen, or the number of patients who ending up needing revisions is significance. I suggest you call your surgeons office and ask to schedule a follow up appointment. Emails aren’t going to fix your problem. You need to be seen by your provider. A dermatologist should easily be able to differentiate a keloid from other skin lesions. If it wasn’t a keloid, it shouldn’t of been injected with steroids.
Simply, following up with the surgeon, who did your procedure is always the first step and that should’ve been taken.
I continue to encourage patients not to travel long distances for elective surgical procedures. If you can’t, or don’t want to follow up with your provider that I suggest scheduling if you in person second opinion, consultations with other plastic surgeons in your community.
