What is this deformity called? Is it fixable and do I need rib cartilages or will ear cartilage work? (Photo)

That particular concern equals to the so called "tombstone deformity"; this is when tip grafting and/or plasties are overly visible under the skin, because the skin was too thin or the maneuver calibrated too aggressive; not sure if you got plasties, grafting or both at the tip, your skin is thick so it must be a matter of recalibrating the technique (in thin skinned tips you need to add superficial temporalis fascia as masking layer).

Based on your photo, and your narrative, I believe that your tip appears somewhat pointy and sharp, because you have thin skin. In that regard, revision rhinoplasty with rib cartilage does not make good sense, unless if you are having structural breathing issues with your nose. Microdroplet Silikon-1000 injections maybe considered for adding permanent volume to your nasal tip area for a less pointy, and less operated nasal tip appearance. I hope this helps. There's not really a particular name of the deformity you are discussing, it really is just that you feel the tip is too pointy/sharp. For fixing it, it really depends on what else is going on. If it's just softening of the tip, I do think you could try to get something done that doesn't involve additional cartilage harvest, like putting fascia there over the tip to soften the edges.

Normally there's what's called a break point of the columella as it approaches the tip of the nose. You have a sharply defined tip with no break point and a very narrow tip. This can be repaired using a graft to to widen and round the tip.

Hello and thank you for your question. Based on your photographs, you are a great candidate for rhinoplasty. Maneuvers for your surgery would include dorsal hump reduction, tip refinement, nasal tip deprojection, tip elevation, among other maneuvers.
