I am sorry to hear about your problems.  It seems your soft tissue is not supporting the weight of the implants. You may need to go smaller still.  With a smaller breast mound, the nipples can be positioned lower relative to the new mound  by excising skin from the inferior pole.   You do have a challenging issue that might ultimately require removal of the implants.  All the Best.

The photos that you posted are not the best to see what is going on. You have several options including placing the implants into a new pocket. It looks like they are on top of the muscle so by going below the muscle you would have less of the " bony appearance and the implant could be placed so the maximum projection is in back of the nipple-areola.
