What is this sag/deformity at the bottom of my butt, and what procedure can fix it? (photos)

I had a BBL a year ago. I never had sag, before. I would it to "curve up at the bottom." Like the wish pic I've attached. Will another BBL by a surgeon who does BBLs often be able to fix it? What exact procedure will? I don't know who to go to, or what to schedule I also would like to get the whole shape fixed more. It's square. And the top where the fat was injected and a shelf was formed, is not blended and looks like implants. Can this all be fixed too? And with what procedure? Thank you.

The dent beneath the "sag" is a very deep attachment to the ischial tuberosities (bony prominencies of pelvis) and that appears first, because of genetics  and then because of the combination of aging and volume added. If you wouldn't have the genetic for it, you most likely wouldn't have the condition. There is a technic which describes a skin and fat "resection" through a vertical incision parallel to the butt crack. Unfortunately Fat grafting will not be very helpful. The "shelf" is fixable. When fat and thus weight are added to the buttocks gravity plays its usual role of pulling things south a bit.   When fat is added to the buttocks a pull towartds the feet is natural.  The bigger the buttocks the more the oull and the more the droop.  This is the simple fact and answer.  My Best, Dr Commons.
