Implant size discussion and implant selection is one of the most important steps during your consultation with plastic surgeon. The only way to get true upper pole fullness is to place an implant. This is commonly done after a lift and provides added volume, projection, roundness and upper pole fullness.

Placing implants behind the muscle will be the best option for upper breast fullness. You do want to avoid going too large.

No is the short answer. In general, any upper breast fullness achieved after a mastopexy will be lost within a few months. It is quite common to place an implant at the time of mastopexy to achieve the results shown in your "wish picture". The good news is that you can easily achieve those results still by placing an implant. I recommend a size of 320-400cc. The implant can be placed under the muscle with a very small incision. It is less common to stage these procedures (mastopexy and augmentation) but it is still done and some would say the results are better with fewer complications. So, no bridges have been burned for you and you are right on track to the "wish picture".
