I would like the line under my butt to be extended. I have seen quite a few before and after photos where the butt is bigger but the line remains the same.I also see photos where it appears to be bigger towards the top of the butt opposed to the bottom and I would rather have the opposite even though even distribution all around is ideal. Could a BBL also be combined with implants? Attached is photos of me& what I’m going for. Let me know if I’m being realistic!
Answer: BBL and implants Dear Ayee.Makay, it is hard to tell for sure without an examination. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations. Answer: Body contouring options A solid surgical plan always begins with a clear understanding of what the problem is. In cosmetic surgery we often called that making an assessment. it is essentially making an accurate and correct diagnosis before prescribing an operation. This begins by first having a clear understanding of what an ideal female figure should look like and then recognizing what is different and how this can be changed. In your case you have a slightly unusual tall pelvic structure giving your buttocks a disproportionate vertical height in balance. your lower extremities are also slightly bottom heavy for your torso. An ideal figure does not necessarily need to add a significant amount of volume. reducing the volume in the love handles with a lower transition zone between your buttocks and waist would create a more harmonious feminine S-curve. fat can be grafted in the hip dip region for better projection in that area if desired. I think in your case well done Liposuction would solve most of the issues which are primarily related to bone as much as fat distribution. Delivering consistent quality liposuction especially knowing how to contour the upper buttocks in the transition zone to the torso seems to be more difficult than most people believe considering their results I see from other providers. Generally speaking delivering consistent quality liposuction and or fat transfer results is more difficult than most people believe and this includes the majority of plastic surgeons. The truth is most plastic surgeons really aren’t that great at Liposuction or fat transfer. even those who claim to be specialists in the field often fail to deliver quality results on a regular basis. For that reason I encourage patience to be highly selective when choosing a provider. Body contouring has more variation in outcomes purely based on who does the procedure than any other operation I’ve ever seen. Finding the right plastic surgeon is absolutely paramount and getting a high incidence of high quality results. To find the right provider I suggest patients have multiple in person consultations starting with local board-certified plastic surgeons. During each consultation ask each provider to open their portfolio and show you their entire collection of before and after pictures. Ask each provider to show as many pictures as possible of patients who have similar body characteristics to your own. An experienced surgeon should have no difficulty showing you at least 50 sets of before and after pictures of it abdominal or mid torso liposuction with or without fat transfer. Highly experienced surgeons will have hundreds or even thousands before and after pictures for this commonly performed procedure. Ask providers to show you examples of excellent outcomes, average outcomes and outcomes that are less than they had hoped for. All plastic surgeons have a variety of outcomes and it’s fair game for patients to ask to see different outcomes. Being shown a handful of preselected pictures which most likely represent the best results of the providers career is totally insufficient to get a clear understanding of what average results look like in the hands of each provider or how many of these procedures a provider has actually performed. Mastering liposuction literally requires doing thousands of cases. In my opinion the procedure is best performed with patients awake using local anesthesia with sedation. The ability to have the patient hold different anatomic positions while contouring their torso is superior to the limited positioning during general anesthesia. Most of the work is best done with patients laying on their sides. this shows body contour better than laying on your back or face down. Positioning patients on their side during general anesthesia is possible but cumbersome and highly unpopular with anesthesiologist and nursing staff. avoiding general anesthesia and doing these as awake procedures for providers will have sufficient skill and experience to do comfortable awake body contouring is in a sense more ideal. Most plastic surgeons are not taught how to do major awake body contouring procedures without general anesthesia during their training and the majority of providers will most likely choose general anesthesia for the procedure. It’s still possible to get good results but in my personal opinion having spent 10 years doing liposuction using general anesthesia and 12 years doing these procedures without general anesthesia I much prefer avoiding patients being put to sleep. Some people have a gluteal crease and some do not. I don’t think the ideal female figure should have one personally. Attempt at creating a gluteal fold or crease when one does not exist is almost always met with failure and disfigurement. it’s far better to work with your current shape on the bottom of your buttocks and enhance and contour the upper part for an ideal elongated torso and balanced S-curve feminine figure. I personally I’ve been doing liposuction as a board-certified plastic surgery for over 22 years. The last 12 years of practice have been devoted exclusively to doing only liposuction and fat transfer work being one of the only plastic surgeons in the country who exclusively specializes in just these two procedures. I’ve personally performed over 8000 liposuction and fat transfer procedures to date. Looking back I recognize that early in my career I thought my skills were above average but in retrospect I recognize that I was far from having reached my potential and the learning curve was far higher and steeper than I ever imagined. This is why I feel I have the authority to say that most plastic surgeons are not particularly good at liposuction and fat transfer. Mastering this procedure requires total dedication to constantly improving outcomes and mastering the procedure. There’s no substitute for doing the same operation on a daily basis seeing patients results on a regular basis to fine-tune technique and skill towards delivering consistent quality excellence. During consultations ask surgeons with the revision rate is and what the revision policy is. Become familiarized with what quality results look like based on reviewing as many before and after pictures online as possible. Create a rating system of quality results that you can implement during consultations. Have pictures of your own torso printed and bring those with you during consultation for reference. A highly experienced surgeon should be able to demonstrate hundreds or even thousands on before and after pictures for commonly performed procedures. Surgeons who are good at Liposuction that generally enjoy doing the procedure and are excited about doing it. I personally do not recommend patients pursue gluteal implants. The procedure is not offered by the great majority of plastic surgeons in the United States for good reason. Complication rates and undesirable outcomes are far too common with using gluteal implants. For patients who opt to have a gluteal implants they should select providers who specialize exclusively in doing the procedure. Only those surgeons can deliver consistent quality outcomes. correctly done gluteal augmentation with implants typically requires intramuscular placement of modest size implants that fit within the gluteus muscle contour. it should only be done by providers who have sufficient skill and experience to deliver consistent quality results and who are in a good position to handle the complications that come from the procedure. I really don’t think you need to gluteal implants and question if you even need fat transfer. Obviously each person has their own unique wishes and desires and this should always be incorporated with a quality assessment. By reviewing sufficient numbers a before and after pictures a clear idea of a working plan with the ability to deliver consistent results should be the end goal. Finding the right provider may require having numerous consultations. There’s no ideal right number of consultations since some of this is simply luck of the draw. Patients can get a certain degree of inference from reviewing plastic surgeons websites. For example, plastic surgeons who simply don’t have a lot of before and after pictures of Liposuction results are unlikely the ones you want to consult with. I highly recommend avoiding virtual consultations whenever possible. Starting with five consultations is probably a reasonable goal and then schedule more if needed. Vetting plastic surgeons is a skill and its self that most patients are not familiar with. Recognized that you are the one in charge and you are the one hiring the surgeon. That puts you in a position of being in control during the consultation and asking appropriate questions and asking for supporting documentation. If you let the surgeon Drive the consultation and steer you towards being one of their patients you missed the opportunity of how to properly screen for the right provider. Most patients take a very passive role during consultations and this is unfortunate. Most patients have not had numerous experiences with cosmetic surgery and find the process potentially intimidating and overwhelming. Quality providers will be detail oriented during surgery and during consultations. Pay attention to how much time is spent with you and how thoroughly providers explain their assessment, review before and after pictures, discuss preparation the procedure and recovery. More than anything else is the ability to get access to a complete set of before and after pictures including a variety of results. In the end there’s no substitute for experience though experience alone is not sufficient to be on top of your game. Consider reading all reviews on various physician review websites avoiding providers for any significant number of justified negative reviews. The absence of justified negative reviews is more important and more telling than having an abundance of positive reviews. Continue having consultations until you feel quite certain you found the best and most appropriate provider for your needs. Listen to what each provider recommends and don’t ask providers to do procedures they don’t agree with. Hopefully patience wishes and desires coincide perfectly with provider recommendations. With careful selection and finding the right provider patients should expect consistent quality results. Unfortunately the opposite happens far too often and considering that Liposuction is a permanent and irreversible procedure patients should take the decision quite seriously. Good luck, Mats Hagstrom MD
