The photograph is very limited, but it appears that you have herniated fat pads in the lower lids. It's important to understand that there are three compartments of fat under each lower lid, and the amount of bags present at age 30 is usually genetic. A trans -conjunctival lower eyelid surgery can accomplish removal of those bags under a brief general anesthesia, which takes about 30 minutes. Anticipate about 10 days of visible bruising and swelling, even though the incision is completely located on the inside of the eyelid.
