The areas you are highlighting appear to be superficial capillaries, and brownish skin discoloration. Both of these skin conditions may be improved with IPL or laser treatments, depending on your examination. Nasal redness tends to occur after multiple rhinoplasties, and this may be treatable. Hi Alejandro! It very well could be hemosiderin staining. If that's the case, you could try IPL to see if improvements can be made. Dissolving with HA will likely just create a divot again, though, if it is the Tyndall effect, you may see resolution of any blue-appearance. I don't believe the area is necrosis and I don't believe a 3rd rhinoplasty would correct the coloration, though the divot could be improved. Cigarette smoking is not ideal for healing as you know, though 1 per day is fairly low. Hope this helps you!
