I really don’t like how round and “bulgy” my eyes look. Does anyone have any recommendations for improving their appearance? Would I be a candidate for orbital decompression or is that too invasive for me?
Answer: Lateral canthoplasty Hello, Thank you for your question and for sharing your photographs. Based on the images, it appears that you have a degree of lower eyelid malposition with lateral bowing. In one of the photographs, there is a slight scleral show when you look upward, but not on straight gaze. Assessment: This type of eye configuration does not specifically require reducing the volume in the eye socket, as this could also lead to the upper eyelids dropping slightly. Instead, there are other interventions that could help achieve a more desired appearance. Treatment Options: 1. Lateral Canthoplasty: This procedure involves changing the attachment at the outer corner of the eye to produce a more almond-shaped appearance. A lateral canthoplasty can help address the round and open appearance of your eyes. 2. Raising the Tail of the Eyebrow- Temporal lift: Sometimes, a lateral canthoplasty can cause a degree of crowding of the tissues in the upper eyelids. Raising the tail of the eyebrow slightly can complement the canthoplasty and improve the overall aesthetic result. I recommend consulting with a specialist who can assess your condition in detail and discuss these treatment options. They can help you develop a personalized plan to achieve the look you desire.

Answer: Bulgy eyes? It does not look that you have true bulgy, protruding eyes. You may have mild bilateral eyelid ptosis? In person consultation with a board certified plastic or oculoplastic surgeon would help. Good luck

Answer: Upper blepharoplasty Unfortunately, you have not provided a lateral view. However, from the frontal view, it appears your upper eyelid creases are quite deep as if you have decreased fat in your globe. You may benefit from an upper blepharoplasty. I do not believe orbital decompression would be indicated. Best Wishes, Gary Horndeski, M.D.

Answer: Facial appearance Facial appearance is primarily determined by facial bone structure. Your soft tissues are normal and your skeletal structure is as well. To get an assessment for eyeball positioning( exophthalmos vs enophthamos) I suggest consulting with oculoplastic surgeons in your community. Personally, I don’t see a good indication for surgical intervention. Best, Mats Hagstrom MD
